Application Form Documents
Use this page to upload the following documents with your application:
• Itemized income and expense budget for the project indicating known and projected sources of financial support.
• Last available financial statement and current year’s operating budget for the organization.
• Organization’s Internal Revenue Service exemption letter.
• A copy of the current documentary evidence from the State of California Franchise Tax Board that applicant is exempt from State Franchise or Income Tax under Section 23701(d), Revenue and Taxation Code.
• A list of name of the organization’s governing board, showing their business, professional or community affiliations. In addition, the names of any member, director, officer or employee of the Foundation associated with the applicant should be listed. Any Director of the Foundation, who is serving as a Member, Director, Trustee, Officer or employee of an organization which is requesting a grant, will abstain from voting on any matter pertaining to the organization with which the common relationship exists.
